Nurse Practitioner (NP) - Medicare Job

NURSE PRACTITIONER (NP) needed for a CONTRACT opportunity without client located in Albuquerque, NM.

THE BIG PICTURE:

- Nurse Practitioner will be providing home care visits for Medical populations.

WHAT YOU'LL BE DOING:

- Provide Home visits to Medicare population. Performs patient physical assessment for Existing conditions, disease Management
- Completes documentation, legibly, accurately and within designated time frame.
- Provides clinical resources and consultation to staff regarding stroke patients with clinical complex needs.
- Prescribe medications as needed
- Works with nurse educators and director to implement formal and informal nurse education and training

WHAT YOU NEED TO BRING TO THE TABLE:

- NM NP (license and national certification, or the ability to obtain national certification and/or NP license in state of assignment) active, unencumbered licensure free of any open/unresolved disciplinary actions including probation or restrictions against privilege to practice
- Prescriptive Authority in state of assignment
- Minimum one (3) year of clinical experience in their highest level of education, clinical setting preferred.
- Family, Geriatrics, or Adult certification required

- Ability to obtain reliable transportation in order to complete home visit assessments
- Excellent administrative, organizational and verbal skills are required
- Professional image and behavior are required
- Strong communication skills with the geriatric or Medicare population
- Ability to navigate on the Internet and work with personal computer
- Ability to navigate stairs and a variety of dwelling conditions and configurations
- Ability to sit, stand and kneel as needed to perform physical assessment.
- Home care or home visit experience is considered is a plus
- Experience in gerontology, cardiology, internal medicine, or endocrinology is a plus
