Nurse Coordinator

I am looking to hire a Nurse Coordinator for our Public/Health Sector client

MAIN DUTIES AND RESPONSIBILITIES:

- To provide quality evidence based assessments, nursing advice and support for service users from the age of 18 to determine eligibility for Continuing Health Care (CHC) and Funded Nursing Care (FNC) ensuring their needs are identified and met in accordance with the National Framework for Client Continuing Healthcare and Funded Nursing Care.
- The post holder will be responsible for assessing the health care needs of service users within acute, care home and community settings.
- The post holder will use the assessment tools of the National framework to assess eligibility of Continuing Health Care/Funded Nursing Care.
- To have a working knowledge of local/national policies and legislation which govern current eligibility for people aged 18 and above, including any other related legislation.
- The post holder will be responsible for case management of case load and ensure timely reviews are completed as outlined in the guidance.
- The post holder will support the quality monitoring of care homes in conjunction with agreed policy and in conjunction with the lead for safeguarding, including supporting any other identified safeguarding issues.
- Identifying and contributing to others training needs and act as a resource to the application of the National Framework.
- Ensuring the promotion of issues of equality diversity and rights in accordance with good practice and legislation.
- Ensuring effective communication pathways exist.

QUALIFICATION:

- Professional Qualification in health (Registered Nursing Qualification)
- Educated to degree level or working towards
- Evidence of continual professional clinical development.

ESSENTIAL EXPERIENCE:

- Experience of continuing health care and the management of complex care patients
- Experience of discharge planning and working organizational boundaries
- Experience of completing needs assessments
