Nurse Case Manager Job

NURSE CASE MANAGER needed for a CONTRACT opportunity with Yoh's client located in Simi Valley, CA.

WHAT YOU'LL BE DOING:

- The Case Manager will be responsible for implementing a collaborative process which assesses, plans, implements, coordinates, monitors and evaluates options and services to meet health needs of patients
- Discuss cases as appropriate with attending/responsible physician
- Document initial utilization and discharge planning assessment on all new patients as needed to support efficient work flow processes within case management
- Obtains authorizations as required by third party payers for patients
- Performs discharge planning services from ED, including home health services, SNF placement and transfer to other facilities

WHAT YOU NEED TO BRING TO THE TABLE:

- Registered Nurse Licensure in State of CA (Active and in Good Standing)
- Bachelor's Degree in Nursing (BSN)
- Two to four (2-4) years of recent clinical experience as a nurse
- Previous experience in Utilization Management or Discharge Planning
- Case Management experience in an Acute Care Setting
- Must have InterQual Guidelines experience
- Basic computer skills and proficiency in MS Word and Outlook required
