Nurse Case Manager Job

NURSE CASE MANAGER needed for a contract opportunity with Yoh's client located in Sacramento, CA.

- Utilization Management Experience

What You'll Be Doing:
- Reviews all inpatient admissions to assure appropriate level of care and identifies members for case management
- Assesses, plans, implements, coordinates, monitors and evaluates individual care plans with physicians, staff, and facilities regarding patient care and provides oversight of this function for the medical group for chronic/catastrophically ill members
- Case manager is also responsible for referrals, follow-up and care coordination of those members eligible for CCS, and identifies alternative cost-effective levels of care when requested care is not covered

What You Need to Bring to the Table:
- Current California RN or LVN license
- Three (3) years recent utilization management or quality management experience in a clinical or managed care setting
- Case Management Certification preferred
- Knowledge of Medi-Cal guidelines and regulations
- Word processing and computer experience required; Microsoft Office Suite experience (Word, Excel, Access) required
