Nurse Case Manager Job

NURSE CASE MANAGER needed for a CONTRACT opportunity with Yoh's client located in Santa Clara, CA.

TOP SKILLS SHOULD YOU POSSESS:

- Case Management Experience
- Clinical Nursing Experience

WHAT YOU'LL BE DOING:

- Support and participate in the interdisciplinary team approach to ensure excellent member satisfaction, effective resource utilization, improved quality of care and cost-effective outcomes.
- Identification of members appropriate for case management programs based on identification of related problems.
- Process requests for services and engage members into case management programs by conducting necessary assessment, completing histories and collecting documentation for the determination of case management program eligibility.
- Perform case management activities as assigned.

WHAT YOU NEED TO BRING TO THE TABLE:

- Current California Registered Nurse (RN) or California Licensed Vocational Nurse (LVN)
- CCM Certification Preferred
- Minimum three years clinical nursing experience in an area such as medical surgical, critical care, home health or skilled nursing
- Two years of experience in case management, utilization management, discharge planning and or quality improvement in a managed care setting
- Proficient in Computers & Microsoft Applications
- Excellent organizational, written, and interpersonal skills
