Nurse Assessor (home Based) - 6-12 Month Contract

An experienced District or Community Nurse is required for a home based role as a CHC assessor (role title: Case Co-ordinator). In this role, you will be reviewing Continuing Healthcare Claims (CHC), providing assessments and feedback. A competitive salary is available (dependent on experience); as a home based role, this opportunity is open to nurses across the UK. The role provides an excellent opportunity to move into the healthcare assessment field of nursing, as a move away from more hands on patient facing nursing. The position is available on a permanent or contractual basis, with a contractual hourly rate of around £28.50 available.

Some key duties include:
*Provide evidence based assessments on all patients within a variety of settings. This may involve liaison with carers, other health professionals, voluntary and statutory organisations and other agencies.
*Act as a point of contact for appropriate CHC issues and facilitate recommendations on eligibility.
*Review care packages to ensure services meet the CCG commissioning contractual requirements.
*Work closely with the CHC Clinical Manager and the Team, producing reports, as required.
*Provide relevant CHC information and effective communication to patients, families/carers and health staff, including appropriate referral to other health professionals.

A Registered Nurse is required with proven experience as a District or Community Nurse. Experience in CHC handling would be beneficial but is certainly not essential. Excellent telephone communication skills and confident computer use is essential. To apply, please contact Ross Taylor on or call .
