LONG TERM CARE NURSE SPECIALIST REGISTERED NURSEneeded for a FULL TIME/CONTRACT opportunity with Yoh's client located in LOS ANGELES, CA.

TOP SKILLS SHOULD YOU POSSESS:

- Registered Nurse in State of CA
- 2-5 years of Case Management and Discharge Planning Experience in a Managed Care HMO Specialist

WHAT YOU'LL BE DOING:

- Responsible for direct supervision and oversight of Long Term Care LVN Nurse Specialists
- Provide a coordinated service delivery system that improves the well-being of members
- Overall Care Management regulatory and accreditation compliance

WHAT YOU NEED TO BRING TO THE TABLE:

- Registered Nurse License in State of CA (Active and in Good Standing)
- Associate or Bachelor's Degree in Nursing Experience
- Graduate from an accredited school of Nursing
- 2-5 years of Case Management Experience in an HMO Managed Care Setting
- 2-5 years of Experience working with Elderly, Disabled and Frail Populations
- Strong clinical background in acute care with at least three (3) plus years of experience
- Intermediate level use of MS Word, Excel and Outlook
- Current regulatory knowledge (CMS and Joint Commission)
- Strong communication and interpersonal skills
