Long Term Care Case Manager Job

LONG TERM CARE CASE MANAGER needed for a CONTRACT opportunity with Yoh's client located in San Mateo, CA.

TOP SKILLS SHOULD YOU POSSESS:

- Geriatrics Nursing Experience.
- Case Management and TARs Experience.

WHAT YOU'LL BE DOING:

- Review and process pre-authorization requests and long term care treatment authorization requests.
- Carry out intensive case management, and individualized care planning in order to facilitate and promote quality, cost effective outcomes and minimize fragmentation of health care delivery.
- Interact with member's family, providers, provider office staff and other administrative personnel to assist member in linkage to healthcare and specialty services.

WHAT YOU NEED TO BRING TO THE TABLE:

- A valid California license as a Registered Nurse.
- Knowledge of Medicare and Medi-Cal benefits.
- Two years geriatric nursing experience and a Bachelor's degree in nursing are preferred.
- Case management experience is ideal.
- Must have own vehicle and valid driver's license with proof of insurance in conformity with state law minimums.

BONUS POINTS! OTHERWISE KNOWN AS PREFERRED QUALIFICATIONS:

- Bilingual skills (Spanish, Tagalog or Chinese) is highly preferred.
- Certification as a Geriatric Clinical Nurse Specialist is a plus.
