We are looking for an experienced Band 3 rehab assistant with experience in inpatient rehabilitation and discharge planning of older people / people with complex needs including continuing Healthcare assessments, mental capacity and best interest decisions.

Start Date: ASAP
Duration:12 weeks
Hours: 2 days per week (Monday & Wednesday)
