Projektbeschreibung

Eden Brown Recruitment require a HCPC registered Leaving Care Social Worker to work within the 18-25 care leavers team.
The successful candidate will be working with young people aged 18 to 25 in order to enhance and further develop their independent living skills, knowledge and abilities working with a variety of stakeholders.

GENERAL DUTIES

1.To provide a leaving care service to former relevant young people aged 18+, including UASC, in line with statutory requirements and departmental priorities.

2.To participate in training as required in the interests of the service overall and of career development.

3.To participate in supervision.

4.To participate in team meetings

5.To participate in emergency duty rota within specific role.

6.To participate in written recording, or using new technology where appropriate.

7.Respond to public and other agencies by telephone, correspondence and direct contact in order to provide a service within departmental priorities.

8.Liaise with relevant external agencies e.g. police, health authority etc.

SPECIALIST CHILD CARE DUTIES –SOCIAL CARE WORKER – 18 Plus LEAVING CARE TEAM

1.To provide a leaving care service to Former Relevant young people aged 18-21 years (or up to 24 years if in continuing education), in line with the requirements of the Children Leaving Care Act 2000.

2.In consultation with each Former Relevant young person, to establish a Pathway Plan which is reviewed every 6 months

3.To undertake ongoing assessments of young people's needs, including Independent Living Skills Assessment and to provide action planning and regular progress monitoring through Leaving Care visits.

4.To regularly liaise with carers and supported housing providers on young people's well being and monitor the safety, suitability and effectiveness of the placement

5.To maintain regular contact with young people in line with the Children Leaving Care Act and service requirements

6.To participate in the emergency Duty Rota for the relevant team, as directed, which will include some work with under 18's.

7.To support young people to access appropriate accommodation

8.To support ex UASC to access appropriate advice and guidance on resolving their asylum status

9.To provide ex UASC whose status has not been granted, with information on other sources/organisations that can provide support with voluntary return to their country of origin.

10.To take into account and listen to the needs and wishes of the young people and involve them in decision making about their lives

11.To actively promote young people's participation in education, employment or training and ensure that they have access to all available opportunities to enhance the quality of their lives
