Inpatient Resource Specialist Job

INPATIENT RESOURCE SPECIALIST needed for a CONTRACT opportunity with Yoh's client located in Fairfield, CA.

TOP SKILLS YOU SHOULD POSSESS:

- Excellent organizational skills
- Previous work history in hospital or medical setting

WHAT YOU'LL BE DOING:

- Will perform a combination of duties to assist with the utilization of hospital resources and coordination of post-hospital care for patients in support of Case Management RN or Social Worker
- Responsibilities include communication and coordination with payers and outside agencies to assist with patient discharge
- Maintain regular communication with Case Managers, Social Workers, appropriate management staff outside agencies and other hospital departments
- Prepare necessary paperwork for the Social Workers and Case Managers
- Provide administrative support to the Case Management Team: faxing, copying charts, follow-up phone calls and generating reports
- Initiates Advanced Directive paperwork with patients as directed by the Case Management Team

WHAT YOU NEED TO BRING TO THE TABLE:

- High school graduate or equivalent required; A.A. or coursework in Social Work or health-related field preferred
- Experience in utilization management, discharge planning, and/or social work preferred
- BSW or LVN preferred
- Two (2) or more years of experience in a hospital or medical setting required
- Experience with Microsoft Word, Excel and Outlook required; PowerPoint, Cerner and MIDAS preferred
- Excellent organization and communication skills
- Excellent interpersonal skills in dealing with co-workers, patients, physicians, and the public at large
