Housing & Transportation Specialist - Social Worker Job

HOUSING AND TRANSPORTATION SPECIALIST/SOCIAL WORKER needed for a CONTRACT opportunity with Yoh's client located in Los Angeles, CA.

WHAT YOU'LL BE DOING:

- Responsible for coordinating access to Home and Community Based Services for members needed to remain living at home
- Coordinates day-to-day interaction between Members and Case Managers
- Working with People with disabilities, senior and low economic status populations

WHAT YOU NEED TO BRING TO THE TABLE:

- Bachelor's Degree is Required; Social Worker or Related Degree
- Three to five (3-5) years of Experience working with people with seniors, los economic status populations and disabilities
- Must have knowledge of housing and transportation service providers in Los Angeles County
- Must have experience with Long Term Services and Support
- Must be proficient with Microsoft Outlook, Word, Excel and Powerpoint
- Must be able to Travel around LA County if necessary (must have clear licensure)
- Long Term Services and Support Experience in a Health Plan
- Demonstrated Knowledge of housing and/or transportation service providers and issues in Los Angeles County

BONUS POINTS! OTHERWISE KNOWN AS PREFERRED QUALIFICATIONS:

- Social Work or related Degree is highly desirable
- Knowledge of Managed Care, Medi-Cal and MediCare is highly desirable
