Qualified Social Worker - Hospital Discharge

Our client has a job vacancy for a Qualified Hospital Social Worker to work within a Hospital Discharge Team based in Merseyside. This vacancy has an initial 3 month contract and the pay rate for this role is £25.00 per hour.

Requirements:
Have you got a social work degree or equivalent?
Are you HCPC registered?
Do you have 2 years experience working within a hospital in the UK?

Responsibilities;
- You will be managing a caseload.
- You will be working within a multi-disciplinary team.
- You will be facilitating safe and timely discharged from hospital.
- You will be undertaking Risk, BIA and capacity assessments
