Home Visitors provide first time expectant or new parents with health and child development information. Through home visits, the goal of the program is to give expectant parents the information and support they need to be prepared for the birth of their child and provide ongoing support for families to maximize this period of development to age 3.

BASIC FUNCTION:

Provide direct services to clients and their families and for related activities.

ESSENTIAL FUNCTIONS:

- Under direction, ensure that the activities of this position and relevant programs are consistent with the mission, vision and values of Catholic Charities, Diocese of Cleveland.
- Engage families and maintain a set caseload as dictated by Ohio Department of Health state rule of parent with children 0-5 years old.
- Determine eligibility of families for program.
- Deliver home visits to eligible families to increase healthy pregnancies, improve their home environment and strengthen parent-child interactions.
- Plan, implement, and facilitate parenting education activities using an evidenced based curriculum.
- Develop realistic and attainable goal plans with family to address child development and social and emotional growth as well as parent/child bonding and positive interactions.
- Review and monitor services, reassess needs and initiate services which as responsive to the needs of clients.
- Assist parents in developing formal and informal community supports.
- Act as an advocate to programs and clients.
- Complete and maintain confidential records and appropriate documentation as required.
- Record data into de-centralized billing system.
- Achieve and maintain credentialing status by completing required training as required by Ohio Department of Health.
- Recruit families into program through community outreach.
- Exemplify positive role modelling by professionalism, language, work ethic, and positive communication.
- Perform other duties as necessary to accomplish objectives.

WORKING CONDITIONS AND PHYSICAL DEMANDS:

- Travel throughout service area required.
- Frequent visits to client's residences and/or attendance at community venues required.
- Must be able to sit, stand, walk, bend, climb, reach, navigate stairs on a frequent basis to conduct home visits.
- Tasks require visual perception and discrimination.
- Ability to work effectively in an office/school environment and at client in-home settings being exposed to a variety of potential hazards including dust, unsanitary conditions, odors, disease, etc...
- Must be able to frequently lift/carry up to 30 lbs. unassisted.
- Reading, writing and finger dexterity to manage data entry and client files required on a constant basis
- Combination of education and experience normally represented by an Associate's Degree in Early Childhood Development, Social Work or equivalent plus 1-2 years related professional experience.
- Must be certified as a Home Visitor by the Ohio Department of Health within 2 weeks from date of hire.
- Must have good oral, written, and interpersonal communication skills.
- Must be well organized, detail oriented, and the ability to maintain confidentiality.
- Good working knowledge of computers and Microsoft Office software.
- Must be able to form good working relationships with various resources.
- Ability to work with diverse client population, community organizations as well as staff at all levels.
- Knowledge of relevant community resources preferred.
- Must have valid Ohio Drivers License and reliable means of transportation for local travel.
- Final applicant is required to be fingerprinted to complete background check.
