HEALTH MANAGER needed for a CONTRACT opportunity with Yoh's client located in Long Beach, CA

THE BIG PICTURE - TOP SKILLS SHOULD YOU POSSESS:

- RN or LVN License for the state of California
- Health/Wellness Education Experience

WHAT YOU'LL BE DOING:

- Will contact new health plan members to conduct telephonic health assessment
- Will work collaboratively with plan members and interdisciplinary health care team to assess, plan, coordinate and provide patient health education to achieve optimal clinical outcomes
- Conduct standardized Health Risk Assessments and other applicable assessments
- Conduct telephonic and face-to-face education to promote health and wellness management
- Coordinate health management activities
- Participate in team meetings

WHAT YOU NEED TO BRING TO THE TABLE:

- Active RN or LVN License in the state of California
- Graduate from an accredited school of nursing
- Bilingual English/Spanish (written and verbal) preferred
- Experience in a healthcare telephonic setting
- Bachelor's Degree in Nursing or related field of study preferred
- Familiarity with NCQA standards
- Effective verbal and written communication skills
- Ability to work well within a team setting
