Health Coach - Registered Nurse Job

REGISTERED NURSE (RN) II/HEALTH COACH needed for a contract opportunity with Yoh's client located in San Francisco, CA.

JOB OVERVIEW:

Under limited supervision, the Health Coach is responsible for providing care coordination and care management services for members. Activities include: assessment, identification, planning, implementation of an individualized plan of care, and evaluation of outcomes. Collaborates with internal and external stakeholders in coordinating a plan of care to achieve optimal quality healthcare outcomes.

THE BIG PICTURE - TOP SKILLS YOU SHOULD POSSESS:

- Three years to five (3 - 5) years clinical setting experience as an RN health coach required (or equivalent experience).
- Managed care experience preferred, preferably in an HMO or IPA setting.

WHAT YOU'LL BE DOING:

- Develops patient outreach activities such as mailings, reports.
- Assist in the development, implementation and evaluation of standards of care, policies, procedures.
- Maintains up to date information on community resources, health coaching literature or recommended screenings and community based programs.

WHAT YOU NEED TO BRING TO THE TABLE:

- Three years to five (3 - 5) years clinical setting experience as an RN health coach required (or equivalent experience).
- California licensed RN or APRN.
- Knowledge of Manage Care concepts, Health promotion strategies, Education background, preferred.
- Excellent verbal and written communication skills.
- Proficiency in Windows-based database applications.
