Duty Worker - Continuing Healthcare

Our client has a job vacancy for a Duty Worker to work within a Continuing Healthcare Team based in Dorset. This vacancy has an initial 3 month contract and the pay rate for this role is £24 per hour (limited company worker).

Requirements:
Have you got a social work degree or equivalent?
Are you HCPC registered?
Do you have at least 3 years recent post qualified experience working within a Continuing Healthcare Team in the UK?

Duties:
- You will be coordinating duty desk activities.
- You will be working with care coordinators.
- You will be arranging packages of care.
- You will be raising safeguarding alerts.
- You will be dealing with health and social care professional referrals.
- You will be responding to crisis telephone calls.
- You will be working within the commissioning strategy process.
- You will be signposting and supporting as need arises.
