Sand Resources is looking for a Discharge/Admission Nurse who has extensive experience within community care services at a senior level.

The ideal candidate will be required to act as the point of contact between community and acute services to identify potential intermediate care patients as well as improving the administration and discharge process into community care.

You will be required to prevent unnecessary admission into acute care while ensuring the transition the community care in a timely and safe manner.

Ideally, you will have experience within the community care setting as well as having extensive experience along admission and discharge processes.

Key Responsibilities:
- To support the MDT discharge decision making process
- To make critical judgements involving highly complex factors
- To prevent avoidable admission to acute care
- To prevent delayed discharge by liaising with community practitioners and Rapid Response Teams
- To enable the process of safe discharge from hospital
- To promote a seamless and multi-disciplined approach to health and social care
- To develop intermediary care services
- To act as a bridge between acute and community care
- To develop risk management procedures
