Complex Case Manager Job

COMPLEX CASE MANAGER needed for a CONTRACT opportunity with Yoh's client located in Mission Hills, CA.

THE BIG PICTURE - TOP SKILLS YOU SHOULD POSSESS:

- Case Management experience
- Utilization Management experience

WHAT YOU'LL BE DOING:

- Will assist in the coordination of care and services provided to members who have experienced a critical event or diagnosis
- Help patients regain optimum health or improved functional capability
- Ensure that services are medically necessary, cost effective and provided in a timely manner
- Telephonic outreach to perform comprehensive assessments of patient's condition
- Timely and accurate documentation of all patient encounters and care

WHAT YOU NEED TO BRING TO THE TABLE:

- Degree from an accredited nursing school required
- Experience in Case Management required
- Current RN license in California
- Current CPR/BLS
- Experience with electronic medical records, and MS Office
