37.5 hours per week- These are to be worked on a shifts rota which may include weekends and bank holidays
Job purpose
To work as part of a Health and Social Care partnership, providing integrated Health and Social care and support to patients and their carers to enable them to be discharged from hospital.
Job requirements
Minimum of 6 months paid experience in a Support/Care environment
Portable DBS accepted
Moving and Handling (Pref but not essential)
Proven track record
Drivers licence and own vehicle required
