Basic Function:
Provide service coordination and related services to participating youth and their families. Service coordination includes family and community support, case management, behavior modifications activities, and on-going case work activities which are considered essential to the family served in gaining access to necessary services. Meet with either the foster, biological or adoptive family in the provision of services to ensure progression to the identified goals and established case plans for permanency. Provide on-going coordination and partnership with collateral, public and private agencies as necessary for family and community stability.

Responsibilities:
- Under direction, ensure that the activities of this position and relevant programs are consistent with the mission, vision and values of Catholic Charities, Diocese of Cleveland.
- Coordinate, gain, access, and/or provide necessary evaluations and assessments including, but not limited to: Psychiatric, psychological, psychosocial, medical social, or vocational that allow the identification of barriers that impede the development of skills necessary for independent functioning in the community.
- Coordinate and participate in the development of the client's individual service plan (ISP).
- Assist family therapist in helping the family define appropriate time limited treatment goals.
- Monitor ISPs and provide service coordination including facilitating referrals to other services.
- Assist and support clients served in crisis situations. Access services from Crisis Intervention Services (typically from organizations designated by the County Alcohol, Drug Abuse & Mental Health Services Board) as needed/appropriate.
- Provide consultation, education and support to family/significant others that are directly and exclusively beneficial to the well being of the client served, as well as maintaining independent living in the community.
- Provide individual interventions for the purpose of developing interpersonal and community coping skills including adapting to home, school and work environments.
- Monitor client's symptoms for the purpose of identifying and minimizing negative effects of psychiatric symptoms which interfere with the individual's daily living, financial management, personal development, school, or work performance. Direct services to aid clients in improving their ability to self-manage such symptoms in community settings.
- Provide assistance to clients served by increasing their social support networks and natural support systems to ameliorate life stresses resulting from the child's disability which may include, if necessary, accompanying clients to activity sites and/or providing assistance in daily living activities.
- Assist
- Combination of education and experience normally represented by a Bachelor's degree in a Human Service field with 2-3 years of case management experience or clinical work with children and families.
- Previous experience in accessing community resources and utilizing community based resources or clinical experience in dealing with mentally ill or SED populations.
- LSW or eligible for licensure preferred.
- Must be at least 21 years of age at time of hire.
- May need a valid Ohio Driver's License, and are subject to a Moving Vehicle Report (MVR) check through the Department of Motor Vehicles depending on program and clients served. Any MVR grade of 'unacceptable' is not permitted to operate a personal vehicle on behalf of Catholic Charities. Must provide a copy of license with reliable means of transportation.
- May need to provide proof of auto insurance including effective dates of coverage and maintain minimum liability insurance coverage of $100,000 per individual and $300,000 per occurrence for driving personal vehicle. This will depend on program and clients served.
- Must be able to work a diverse schedule that may include evenings, weekends, or holidays.
- Must be able to form good working and therapeutic relationships.
- Must have a working knowledge and utilization of community resources.
- Must have an understanding of family dynamics and a commitment to family preservation principles.
- Must be sensitive to the cultural needs of clients served and their families.
- Final applicant is required to be fingerprinted to complete background check.
