Community Psychiatric Nurse - Chelmsford

Our client has a vacancy for a Community Psychiatric Nurse to work in Chelmsford. The pay rate for this role is 21-26 per hour.

Duties -
- To be responsible as the Care Coordinator for the assessment and coordination of care needs, planning and implementation of evidence based care to a defined group of service users.
- Ensuring that assessments, education and support is delivered in accordance with the requirements of NSF.
- Coordinating the care and assessment under the Care Programme Approach of a designated caseload.

Requirements -
- All applicants must have a RMN first level registration, valid NMC registration.
- A minimum of 2 years post qualifying experience within a similar role.
