Community Nurse (Band 5)

Location: Allington Clinic, Ipswich

Hours: 37.5 hours (Monday-Sunday) Fixed Term Contract 12 months

Salary: 21,478-27,901 per annum

We are looking for a Community Nurse (Band 5) within Suffolk Community Healthcare on a fixed term contract for 12 months.

Working in the community and in-patient settings you will be responsible for the following:

- Manage a defined caseload of patients / clients within the Local Health Care Team locality, in-patient setting, day and treatment, community.
- Assess patients` needs and make appropriate plans for care, implement care plans and evaluating effectiveness.
- To contribute to own development and the development of others, working in accordance with the philosophy for rehabilitation, and NMC Code. - Work as part of a multi-disciplinary team
- Participate in internal rotation including weekends and bank holidays

Essential skills & experience

* Previous relevant nursing experience, ideally within the community
* Level 1 Registered Nurse
* Current NMC registration
* Demonstrate a knowledge of Healthreward praise and good practice; challenge poor practice; and escalate concerns readily. (OPCE) will support the elimination of poor practice. This is everybody`s business no matter what your grade or role might be within the healthcare workforce."
