Community Nurse Band 5

Serco Suffolk Community Healthcare is an innovative and transforming provider of community health services within Suffolk. Our vision is for a flagship service to support the shift of higher acuity care from acute hospitals into the community setting. This includes clinical interventions that will prevent hospital admissions and that will facilitate patients being cared for earlier in their recovery following hospital admission in the community setting.

An opportunity has arisen for a staff nurse to join the community health team based in Haverhill, Suffolk on a fixed term basis.

The team aim to provide a holistic nursing service for the residents of Haverhill and the surrounding area. Patients are seen in their own homes and input is also provided into the residential homes within our area.

The workload is varied and includes provision of palliative care as well as meeting other nursing needs. The team works closely with the therapy team and specialist nurses within the organisation and other external agencies such as the GP`s, social workers and care agencies. They also have good links with other local services such as the hospice.

This role is for a fixed term of 6 months

RGN Qualification

Current NMC Registration.

Extensive post registration experience

Enthusiastic and well motivated

Able to work across sites

Car driver
