Community Mental Health Practitioner - Older People

Our client has a job vacancy for a Community Mental Health Practitioner to work within an Older Peoples Mental Health based in Hampshire. This is a full time position with an initial 3 month contract and the pay rate for this role is 20-24 per hour (limited company worker).

Duties
- Co-ordinating care.
- Managing an agreed caseload of service users.
- Formulating care and risk management plans.
- Undertaking initial assessments.
- Providing recovery focussed therapeutic interventions in line with agreed care plans and risk assessments.
- Monitoring and reviewing the effect of prescribed medicines on the patient group.
- Administering prescribed medication.

Requirements
- Professional nursing qualification at Diploma/Degree level i.e RMN and a valid NMC Pin.
- Relevant post registration qualifications/experience.
- A minimum of 2 years post qualifying experience working within a similar role.
