Community Based Practice Nurse x2 (12 Month Contract)

Hourly Rate £19.00 - £ 22.00 per hour

We are looking to appoint into 12 month contract posts - 2 x Community chronic disease nurses, these posts are full time Monday to Friday and will be working within the Yorkshire regions.

The aim of the role is to reduce in-equalities in primary care, by ensuring that those individuals within the community have the same access to quality holistic care and chronic disease management as does the practice population who are able to attend surgery. (use of a vehicle will be essential to the role )

Target Areas:-

Asthma / Atrial Fibrillation / Cardiovascular Disease (CVD) / Heart Disease / Chronic Kidney Disease / Chronic Obstructive Pulmonary Disease (COPD) / CVD- Primary Prevention
Dementia / Depression / Diabetes (including- Glucose Intolerancesmoking status, alcohol consumption, dietary habits, exercise and mobility levels and independent living status.
Sexual health advice will be offered appropriately.
Immunisations will be offered as per Health Protection Agency, Department of Health and NICE guidance.
Family history to be noted; CVD, hypertension, Diabetes, Respiratory disease, Cancers
Primary prevention screening CVD; NHS health checks / lipid modification DIABETES
Obesity; weight management, local health wise initiatives COPD; significant smoking pack year History, recurrent chest infections, asthma diagnosis Cervical cytology Contraception and Sexual Health Lifestyle advice / smoking cessation Immunisations; childhood / HPV / teenage / influenza / pneumococcal / travel.
