JOB SUMMARY

My client, an acute NHS Trust, is embarking on an exciting programme of work with its health and social care partners across our region to develop a new kind of joined-up health and social care service.

The Change/Transformation Manager will engage with clinical and operational leads to gather the Trusts requirements for the Integrated Care Record System. The vision of The Integrated Care Programme is to deliver radical changes in the way people look after their own health, the way they access health and social care services and the way in which local services are organised, including the possibility of a new integrated care organisation. The Integrated Care Record System will build a platform for sharing health and social care information and a key part of this role is to understand the initial requirements as well as supporting the wider programme.

MAIN DUTIES AND RESPONSIBILITIES

To gather the Trusts requirements for the system

To produce a transformation plan and deployment strategy and deliver against it
-

To capture and document current and future state processes
-

To develop new processes, protocols and policies to support change management work arising as part of the deployment
-

To identify benefits and ensure the project is on track to deliver them
-

To ensure that outputs are of high quality and produced on time and within budget, in line with the requirements outlined in the project plan
-

To proactively monitor programme progress, issues and risks, taking corrective and mitigating action where necessary
-

To effectively manage a variety of stakeholders, typically multi-agency in nature, with conflicting interests and priorities
-

To undertake patient and staff engagement, as required and/or outlined within the project plan
-

To provide regular feedback and update reports on progress, issues and risks to senior stakeholders

PERSON SPECIFICATION, KNOWLEDGE, EDUCATION AND EXPERIENCE

- Experience of working with clinicians to lead and manage change across a number of care settings
-

Experience of programme/project management in a healthcare setting, with a track record of delivery and successful outcomes
-

Educated to degree level or equivalent experience

SKILLS

Strong verbal and written communication skills - ability to produce clear and concise written reports and deliver feedback and/or presentations to a range of stakeholders on complex and contentious matters
-

Ability to build collaborative and effective working relationships at all levels, both within and outside of an organisation
-

Ability to challenge and influence a range of internal and external stakeholders
-

Ability to analyse and interpret quantitative and qualitative data, using this to draw robust conclusions and recommendations
-

Ability to manage competing priorities and work under pressure to meet deadlines
