Clinical Pharmacist Job

CLINICAL PHARMACIST needed for a TEMPORARY opportunity with Yoh's client located in San Francisco, CA.

TOP SKILLS YOU SHOULD POSSESS:

- California Pharmacist License
- Experience with Medi-Cal
- Experience with Medicare Part Dl

WHAT YOU'LL BE DOING:

- Clinical Pharmacist will provide pharmacotherapy expertise in support of the Pharmacy and Therapeutics (P&T) Committee, and drug formulary, prior authorization, drug utilization review, and physician education programs.
- Clinical Pharmacist will be part of the Prior Authorization Process: Perform clinical review of PAs/MRFs.
- Clinical Pharmacist will take part in the Pharmacy Review Committee: Prepare for bimonthly or monthly meetings (new drug reviews for CA and Medi-Cal formularies, drug class reviews for CA).
- Clinical Pharmacist willa also participate in the Clinical Pharmacy Program - CPOP, MTM-LTC, Pharmacy Case Review, CMS Star Rating Clinical Initiatives. Pharmacy Case Review of High Utilizers, Narcotic Utilizers, Adverse Drug Interaction Reports, Poor Medication Adherence Reports. Clinical Initiatives Related to CMS Part D Plan Star Rating Help with pharmacy claims adjudication, benefit and formulary problems.

WHAT YOU NEED TO BRING TO THE TABLE:

- Licensure as a Pharmacist in the State of California.
- PharmD from an ACPE accredited college of pharmacy program is preferred.
- Equivalent to two years work in a clinical pharmacy.
- Therapeutic drug utilization.
- Medicare Part D and Medi-Cal.

BONUS POINTS! OTHERWISE KNOWN AS PREFERRED QUALIFICATIONS:

- Managed care and drug information center experience is a plus.
