Certified Case Manager (RN) Job

REGISTERED NURSE - CASE MANAGER (RN/CCM) needed for a CONTRACT opportunity with Yoh's client located in San Diego, CA.

The Big Picture - Top Skills Should You Possess: RN Case Manager
- Perform full range of duties within the general description of Case Management, following provided company protocols with productivity standards clearly stated.
- Document all care in a timely manner established by company protocol, preparing designated reports.
- Remain calm while performing under emergency conditions and anticipate potential problems, and take appropriate measures to avoid those.

What You'll Be Doing:
- Best qualified candidate will be able to assist with designated duties, including but not limited to, on call for hospital admissions, practice aseptic techniques and participate in the implementation of the Family Advocacy Program as directed.
- Also, attend annual renewal of training such as family advocacy, disaster training, infection control, sexual harassment, and bloodborne pathogens, as well as performance improvement detail.

What You Need to Bring to the Table:
- Best qualified candidate will have two to three (2-3) years of experience dealing with fast paced work environment, accurately checking in and entering patient information.
- Furthermore, that candidate will understand the aspects of managing the scheduling for appointments and follow up scheduling as needed.

Bonus Points! Otherwise Known As Preferred Qualifications:
- Utilize automated systems such as CHCS, AHLTA, ADS, and Clinical Information System (CIS)
- RN License In CA
- CCM/CDMS/COHN
- Master's Degree in Nurse Case Management preferred but not required
- CPR Certified
