Due to Increased demand from our clients, Your World Healthcare are currently recruiting for an Experienced Case Manager with Community Aged Care experience. 

Responsibilities:  

- Demonstrate an advanced level of psychosocial assessment and intervention skills.
- Demonstrate advanced social work reasoning skills that enhance the service's ability to manage more complex clients within the clinical specialty.
- Provide social work clinical services to client groups requiring advanced practices kills and clearly articulate these to others within the team. Demonstrate the application of specialised social work knowledge within the clinical specialty when performing novel, complex or critical tasks.
- Exercise and articulate independent social work professional judgment when required in solving problems and managing cases where principles, procedures, techniques and methods require expansion, adaptation or modification.
- Performing as an active member of the Social Work team and contributing to building a positive professional culture that facilitates effective patient outcomes in a constantly changing environment is a major challenge for the position holder.
- Working with clients who may be experiencing high levels of distress, anger and anxiety. Prioritising competing demands in crisis situations.

Must have:

- Relevant tertiary qualifications in nursing or allied health profession 
- Case management and aged care experience 
- High level understanding of the needs of care recipients with complex needs 
- A commitment to best practice in relation to the provision of quality care 
- Superior written and verbal communication, organisational and computer skills 
- Strong leadership and staff management skills 
- Current drivers licence
- Own Car
