Our client is a leading NFP organisation who has a reputation in responding to those in need. They are committed to providing high quality services to enhance the well-being of their patients.

An exciting opportunity exists for an experienced Community Case Manager within Aged Care. This is initially a full time fixed term (12 months) maternity leave position with the scope for a permanent opportunity.

As the Community Case Manager, your ROLE is to:

- Ensure work practices are consistent with all legislation and regulations
- Provide a complete assessment on clients and identify their needs
- Plan the necessary care and support needed for the client
- Monitor and report on client's progress to ensure objectives are met
- Evaluate the outcomes and identify opportunities for improvement

REQUIREMENTS:

- Current registration with AHPRA
- Current Driver's license
- Experience in Case Management, Community Care and Aged Care is desirable
- Australian Work Rights
