CARE MANAGER needed for a CONTRACT opportunity with Yoh's client located in Miami, FL.

- Managed Care Experience
- In depth knowledge of InterQual

WHAT YOU'LL BE DOING:

- Responsible for health care management and coordination of members in order to achieve optimal clinical, financial and quality of life outcomes
- Work with members to create and implement an integrated collaborative plan of care
- Coordinate and monitor member's progress and services to ensure consistent cost effective care
- Promote integration of services for members including behavioral health and long term care to enhance the continuity of care for members
- Conduct face to face or home visits as required

WHAT YOU NEED TO BRING TO THE TABLE:

- Current Florida RN or LPN license; or Social Worker
- Bachelors Degree in Nursing or Social Work
- Case Management experience required (in Clinical, Hospital, or Managed Care settings)
- Knowledge of ICD-9, CPT coding and HCPC
- Knowledge of SSI, Coordination of benefits, and Third Party Liability programs and integration
- Familiarity with NCQA standards, state/federal regulations and measurement techniques
