New Horizons Care Limited are a unique organisation that provide specialist complex care services to adults & children with a neurological condition that may include acquired brain injuries, spinal injuries, or progressive diseases such as dementia, multiple sclerosis and motor neuron disease throughout the Leicestershire, Northamptonshire and Rutland areas. Support is also provided to adults and children with learning disabilities and autism which enables them to access the local community and take part in leisure and respite.

JOB PURPOSE

To develop existing and new care packages ensuring that they are client focused, financially secure, quality driven and professionally managed. You will meet the changing needs of our clients and regulatory authorities by managing care packages effectively and delivering a high quality customer/client service.

MAIN RESPONSIBILITIES

- Incorporate the new Health & Social Care Act 2008 and CQC Essential Standards in your working practices.
- To comply with relevant legislation, operational guidelines and policies/procedures of New Horizons Care Limited.
- To obtain, read and evaluate background medical, therapy, educational, employment and social reports.
- To co-ordinate and ensure continuity of care from the initial care package assessment through to integration.
- To facilitate the interviewing and recruitment of staff teams for each care plan.
- To ensure provision of appropriate equipment, clinical disposables and the funding stream applicable to these requirements.
- Liaise with community practitioners, therapists, medics and the like to ensure the best possible outcomes for your clients.
- To ensure you evaluate on an on-going basis the client's psychological, emotional and physical well being.
- To maintain professional confidentiality at all times and ensure that the client records are truthful, accurate and up to date.
- To maintain good interpersonal relationships with all team members, multi-disciplinary team members that ensures effective communication.
- Lead and motivate your Support Workers in order for them to meet the required standard of care within each care package.
- To achieve KPI's on a monthly basis ensuring that all relevant meetings are conducted in a professional and timely manner.

QUALIFICATIONS/EXPERIENCE

ESSENTIAL:- - Awareness of psychological aspects of supporting clients with disabilities

- Able to travel within the Leicestershire and Rutland areas.
- Passionate about the Health & Social Care sector.

DESIRABLE:- - Experience of managing clients within the health and social care industry.
- Management or Health Care qualification - NVQ Level 4 or a nursing qualification.
