Care Management Coordinator Job

CARE MANAGEMENT SPECIALIST needed for a Contract opportunity with Yoh's client located in Los Angeles, CA.

WHAT YOU'LL BE DOING:

- Support the Care Management Specialist for Adult Services
- Handling the administrative functions of the care management process
- Performs data entry and processing of health risk assessments in the system
- Collects information required by clinical staff
- Support Care Management Specialist for Adult Services
- Intake, logging, tracking and status follow-up

WHAT YOU NEED TO BRING TO THE TABLE:

- High School Diploma
- Medical Assistant Certificate
- At least one (1) year of experience in Medi-Cal managed Care Authorization processes
- Knowledge of Medical Terminology
- Knowledge of ICD-9 and CPT Codes
- Proficient with Microsoft Word, Excel and Access
- Strong Verbal and Written Communication Skills

BONUS POINTS! OTHERWISE KNOWN AS PREFERRED QUALIFICATIONS:

- Associate or Bachelor's Degree preferred
- Knowledge of MHC and CSIIM computer System
