RESPONSIBILITIES:

Responsible for collaborating with the interdisciplinary medical home care team in the provision of patient care, for performing a clinical review of selected patients to determine service needs, and for coordinating the care of selected patients assigned to the medical home care team and primary care operations. In addition, the role integrates and coordinates access and utilization management, proactive patient panel management, care facilitation and treatment planning functions. Upholds the standards of the system-wide customer service program.

QUALIFICATIONS:

REQUIRED:
Valid Ohio RN licensure. BSN or obtains within 2 years of hire. Minimum of five years clinical experience. Ability to work independently and as a member of a multidisciplinary team. Demonstrates excellent interpersonal, verbal/written communication and delegation and problem-solving skills. Demonstrates strong analytical, data management and computer skills. Demonstrates strong leadership, organizational and time management skills

PREFERRED:
Bachelors of Science in Nursing and/or Masters Degree Two years experience with case management, access management, home-health care, discharge planning or related experience. Case Management Certification Experience with case management, access management, home health care, psychiatric case management, discharge planning or related experience. Knowledge of EPIC functionality. Knowledge of case management software.
