BILINGUAL MASTER OF SOCIAL WORKERneeded for a FULL TIME/CONTRACT opportunity with Yoh's client located in Los Angeles, CA.
TOP SKILLS SHOULD YOU POSSESS:

-Master of Social Worker (MSW) in State of CA (Active and in Good Standing)
-Must maintain current CPR/First Aid Training
-Must be Bilingual in Spanish
-2-5 years of Case Management experience, specifically creating care plans; knowledge of physical, mental and social needs of the elderly

WHAT YOU'LL BE DOING:

-Responsible for Utilization Management, Quality Referrals, and Discharge planning in a Pace Elderly Setting
-Demonstrate the knowledge and skills necessary to provide appropriate case management
-Perform psycho-social assessments. Contact family and past mental health providers for histories

Assess discharge-planning needs of a diverse cultural and ethnic population -Assigned patients ranging Adults to geriatrics- must have experience
-Must be flexible between hours of 8am-8pm

WHAT YOU NEED TO BRING TO THE TABLE:

-Master of Social Worker (MSW) in State of CA (Active and in Good Standing)
-or BSW with 2 years' experience
-2-5 years of Case Management experience, specifically creating care plans; knowledge of physical, mental and social needs of the elderly.
-Must have 3-5 years of Clinical Experience
-Bilingual in Spanish is a Must
-Knowledge of DHCS, OSHA, safety regulations and HIPAA Guidelines including the PHI privacy requirements
-Intermediate level use of MS Word, Excel and Outlook
-Strong communication and interpersonal skills
