Band 6 Community Psychiatric Nurse - Older Peoples Community Mental Health

Our client has a job vacancy for a band 6 community psychiatric nurse to work within older people community mental health team based in Devon. This vacancy has an initial 3 month contract. The pay rate for this role is 20 - 22 per hour (limited company worker).

Duties:

* Working alongside other members of the older peoples outreach Service and community mental health team in the provision of outreach support to older people on the OPCMHT caseload in their own homes and communities who may benefit from a recovery focussed approach and additional support
* Managing a client caseload within a defined clinical area of the community mental health team and promote positive mental health
* Providing safe, sound and supportive mental health services by joint working within a multidisciplinary setting
* Coordinating the work of the other members of the OP outreach team, ensuring that they are able to provide defined and agreed interventions within the community
* Communicating and collaborating effectively with care coordinators and all agencies and agents involved in supporting older adults with severe mental health problems
* Contributing to all health and social care assessments for those clients on the outreach day service caseload as prescribed under the CPA/care management procedures.
* Being responsible for the flexible management of a defined caseload designed to meet the individual needs of the client and the carers
* Maintaining high standard of individualised client care, recognised factors and events that are likely to affect the mental, emotional and physical well-being of clients/carers and provide advice and support when required
* Working alongside the care coordinator in assessing clients' needs and strengths through a collaborate approach, ensuring that all clients have care plans and that these are implemented and regularly reviewed/updated
* Developing and maintaining a high standard of practice, based on assessment, through the use of a range of psychosocial approaches, within a health and psychosocial framework
* Communicating/liaising effectively within relevant agencies to ensure that an integrated programme of care is provided throughout
* Being able to manage medication in accordance with the trust policy and the NMC standards for administration of drugs
* Organising and participating in the admission, discharge and transfer of clients in accordance with the trust policies
* Providing clients and carers with verbal and written up to date information on the services offered by the trust, how to express their views, complaints procedure and what they can expect from the outreach team
* Attending weekly OPCMHT clinical meetings to provide the team with relevant information in relation to caseloads and to give feedback on new assessments
* Supervising and monitoring the therapeutic effects and side-effects of prescribed medication providing the client with clear advice and information
* Developing and maintaining effective working relationships with in-patients facilities regarding admission and discharge of clients
* Facilitating carers' assessments, provide the appropriate care plan to meet identified needs and to monitor efficacy

Requirement:

* All applicants must be a Registered Mental Health Nurse (part 3 or 13) with a valid NMC registration.
* Applicants must have at least 2 years recent post registration experience working within mental health.
