Band 6 Community Psychiatric Nurse - Gloucester

Our client has a job vacancy for a Band 6 Community Psychiatric Nurse to work in Gloucester. This vacancy has an initial 3 month contract and the pay rate for this role is 23-26 per hour (limited company worker).

Duties:
* Being responsible as the Care Coordinator for the assessment and coordination of care needs, planning and implementation of evidence based care to a defined group of service users and their carers within a defined catchment area/locality.
* Providing professional advice and support to other agencies and members of the multidisciplinary team, and act up for the Team Manager in there absence as appropriate.
* Acting as a role model and resource for junior/less experienced staff.
* Taking responsibility for the assessment planning implementation and evaluation of nursing care within agreed parameters, and have a working knowledge of local, national policies and legislation, which govern current service provision and audit.
* Being responsible for the assessment, planning implementation and evaluation of the evidence based care required, including health promotion for a named service user/group.
* Taking responsibility for coordinating the care and assessment under the Care Programme Approach of a designated caseload.
* Being responsible for the promotion of carer and service user involvement within the service and for its provision.
* Taking responsibility for establishing therapeutic relationships with service users, and the implementation of evidence based therapeutic interventions with appropriate boundaries in accordance with Nursing and Midwifery Councils professional code of conduct.
* Being responsible for ensuring that carers assessments, education and support is delivered in accordance with the requirements of the NSF.

Requirement:
* All applicants must be a Registered Mental Health Nurse (part 3 or 13) with a valid NMC registration.
* Applicants must have at least 2 years recent post registration experience working within mental health.
* Understanding by the post holder of their responsibilities under the Mental Health Act 1983.
