Band 5 Early Intervention Community Practitioner - Cumbria

Our client has a job vacancy for a Band 5 Early Intervention Community Practitioner to work within Cumbria. This position has an initial 3 month contract and the pay rate for this role is 20-23per hour (limited company worker).

Duties -
- Working collaboratively within an assertive approach to encourage active engagement with service users and their families.
- Working as a proactive and supportive member of the multi disciplinary team.
- Contributing to team training and development sessions by taking a lead in training delivery.
- Maintaining effective implementation of the Care Programme Approach and other legislative procedures.
- Actively identifying, assessing and implementing care plans for clients and carers.
- Working in partnership with Children' Social Services and supporting local safeguarding procedures.
- Acting as the link practitioner for specific CMHT's in promoting the service and facilitating referrals and assessments

Requirements -
- All applicants must have a RMN first level registration, valid NMC pin.
- A minimum of 2 years post qualifying experience working within a similar role.
- 1 years experience working with children & adolescents.
