Band 5 CPN within Older Peoples CMHT - Hull

Our client has a job vacancy for a Band 5 Community Psychiatric Nurse (CPN) to work within Older Peoples CMHT in Hull. This position has an initial 3 month contract and the pay rate for this role is 15-20 per hour (limited company worker).

Duties -
- To work alongside other members of the Older Peoples Community Mental Health Team in the provision of outreach support to older people on the OPCMHT caseload in their own homes and communities who may benefit from a Recovery Focussed approach and additional support
- To manage a client caseload within a defined clinical area of the Community Mental Health Team and promote positive mental health. To provide safe, sound and supportive mental health services by joint working within a multi-disciplinary setting
- To co-ordinate the work of the other members of the OP Outreach Team, ensuring that they are able to provide defined and agreed interventions within the community
- The post holder will communicate and collaborate effectively with Care Co-ordinators and all agencies and agents involved in supporting older adults with severe mental health problems

Requirements -
- All applicants must have a RMN first level registration, valid NMC pin.
- A minimum of 2 years post qualifying experience working within a similar role.
