Our clients based in North London require a band with proven experience & skills in mental health. The remit of the team is to provide integrated care and care co-ordination for service users with complex needs to enhance quality of life, improve access to primary care services, improved physical health and to ensure people in 24 hour support receive a high standard of support which actively promotes recovery and rehabilitation.

Hours: 15hrs per week
