Assistant Team Manager | Adults | Borehamwood

We are recruiting an Assistant Team Manager within Adults Services in Borehamwood, Hertfordshire. Pay rate is up to £35/hr.

The successful candidate will be required to manage Homefirst alongside the flexible Hospital Service. An important part of the role is being aware of risks at all times and reviewing how they can be minimised.

You will have the ability to assess the needs of people referred to the Home first team for social care support and to take appropriate actions to meet this need, with particular focus on prevention of hospital admission. Identify risk, plan and implement care, carry out safeguarding investigations (with the support of a manager), promote independence and complete support planning and brokerage with service users to ensure that outcomes identified at assessment are met within a personal budget. You will be community focused and confident in building professional relationships as well as supporting and advising service users and colleagues.

You should hold a recognised Social Work Qualification, be HCPC registered and be a car driver with access to your own vehicle.
