OCCUPATIONAL THERAPIST -PERMANENT PART TIME POSITION. (0.6 FTE)

The position is in the ASET Team in emergency assessing people aged 70 years and over following the ASET Guidelines who present to Accident & Emergency.

The OT role is to assist the team make decisions about the care needs of the patient, including the need for admission and to make referrals to other services as required. The OT also assess the patients functional status and organises equipment and minor modifications to facilitate discharge from A&E when appropriate.

The role may include assessing other patients in A&E and facilitating discharges from acute beds when required.

- To assess patients presenting to accident and emergency in accordance with the ASET Guidelines
- Document results of patient's assessment, treatment and progress within ASET, Departmental and Hospital guidelines.
- Participate in patient care and discharge planning with the multi-disciplinary team in the A&E.
- To assist with discharge planning from the Acute wards when hospital is at capacity to facilitate admission from A&E
- To provide individual Occupational Therapy functional assessments for patients in the Emergency Department to determine appropriate care plan.
- To coordinate OT intervention in consultation with other members of the multi-disciplinary team.
- To provide equipment and assistive devices for use in the home to maximise a clients independence with ADL's for patients being discharged from A&E
- Conduct home assessments to implement home modifications or adaptive equipment where appropriate for patients discharged from A&E
- Identify own continuing education needs in consultation with the Occupational Therapy Manager or Team leader.
- Identify and become involved in Quality Improvement activities when appropriate.
