Advanced Practitioner - Older People Team - £30 Ph Ltd

We are looking for a Advanced Practitioner for a fast paced first response service, you must be able to problem solve, prioritise quickly, identify risk, be an experienced safeguarding practitioner as well as be able to supervise a team.

The Role
Advanced practitioner locums (25 - 30 hours a week) will be supporting the Community teams to manage their additional staff including the Meals On Wheels reviews and assist with the increased authorisations thus supporting the team managers during the next 4 months.

The client's staff has high caseloads – and do not have the capacity to take on the reviews without some dedicated support in the interim.

Job Purpose

To support the community teams by helping them manage their staff. Helping reduce the pressures of the rise in case's and reviews.
You will be responsible for
Supervising comprehensive multi-disciplinary assessments under the NHS and Community Care Act 1990, of people living in the community of Luton and using a person centred approach to identify eligible needs.
Supporting social worker who are carrying a caseload of service users (older people and people with a physical disability) and through the Care Management process provide advice, support and professional expertise with the aim to promote independence and maximise potential. Part of this caseload will involve long term professional support for a number of very vulnerable people whose condition is complex, unstable or deteriorating and/or where there are complex support network dynamics.

Candidate minimum requirements:
Enhanced DBS
HCPC registered
Car driver/ owner
Senior social worker experience in adult service is a must
