Projektbeschreibung
The role will include full duty work- receiving urgent allocations and undertaking rapid assessment and support planning. There will also be a great deal of multi-agency work, Close partnership working in a fully integrated multidisciplinary team including community matrons, rehabilitation nurses, therapists, AMHPs, CPNs, voluntary sector representatives and clinical psychologists. Cases will be presented to a multidisciplinary group meeting on a weekly basis.
To apply for a role, you must have a minimum of 2 years post qualified experience working in Adults Services in a statutory capacity, be HCPC registered and hold a relevant Social Work qualification.
