Length of Assignment: Full-Time Contract, until March 31, 2014
Description of Project or Company/Organization:
Calian is pleased to be a part of Canada's healthcare industry. Our dynamic healthcare teams consist of more than 500 professionals providing healthcare services coast-to-coast at over 30 locations in Canada.
Through our Health Service Support Contract with the Department of National Defence, Calian's healthcare professionals work in partnership with military healthcare teams providing primary care to today's Canadian Forces.
Job Description:
- Conducts comprehensive case management (CM) assessments using the case management process. Provides client centered CM services to assist client's return to active duty (Return to work Program) or to civilian life.
- Coordinates and monitors clients engaged in the case management program.
- Consults with other professionals to ensure a comprehensive plan is developed.
- Coordinates and tracks benefit applications with various departments and agencies such as VAC, HRDC, SISIP and others; follows up to ensure benefits have been processed.
- Conducts presentations to promote the CM program to units, internal and external agencies.
- Collects data about individual CM workloads, types of clients, pension applications and client satisfaction and forwards the information to the national office on a regular basis.
Mandatory Qualifications: - Candidate must be licensed as a Registered Nurse in Ontario
- Knowledge of military medical and administrative systems an asset.
